ACO suport + minor bugfixes

Initial support for reading and writing Adobe's aco format, used by Photoshop and other Adobe products, has been added.
Bugfixes:
Color entries now display the correct border color when created.
Files are properly closed when reading GPL and PAL formats.
Fixed crash when reading a GPL palette with an empty name.
